Hours: Full-Time. Office hours with some evenings required. 
 

Primary tasks:

   » Conduct outreach and engagement activities: participate as part of a team to provide outreach to the Latino community with the goal of assisting individuals with symptoms of mental illness to connect with needed services.  Serve as a liaison between individuals, CLUES, and Ramsey County Human Services. Reach out to individuals in a variety of community locations.  
   » Provide case management: identify needed services, assist individuals in accessing services (including transportation), advocate on behalf of individuals served to ensure that needs are met. Visit individuals in their homes. Work within the context of the client and his/her family.
   » Provide education regarding mental illness and services available to individuals, families or groups. 
   » Provide crisis intervention and/or plans for clients and their families. 
   » Maintain up-to-date client records, including case notes. 
   » Coordinate data collection and program reporting.
   » Maintain client confidentiality in conformity with HIPAA, agency policies and other applicable laws, regulations and ethical standards.
   » Interact with multi-disciplinary team (clinical and social work professionals) to support the clients Provide input and feedback to refine the Creando Puentes Program, the development of the team, and the processes involved.
   » Cultivate working relationships with service collaborators, and other appropriate stakeholders.
   » Assist in the delivery of contracts results and relationships with contract stakeholders. Participate in all internal and external activities as required by program contracts.
   » Participate in program meetings, regular department meetings; agency “All Staff” meetings, and other meetings as assigned.
   » Promote the mission, operating values and agency services of CLUES within the community.
   » Carry out all other tasks as assigned by manager.

practice:

Education, practice & Key Abilities

   » Bachelor’s degree in social work, Psychology, Counseling, Public Health or related field or equivalent combination of education and practice required.
   » Master’s in social work, Psychology, Counseling, Public Health or related field preferred.
   » practice in providing case management services preferred. 
   » practice providing outreach/engagement services in the community preferred. 
   » practice working with individuals with symptoms of mental illness preferred.
   » Bilingual in Spanish & English with cultural sensitivity and consciousness required. 
   » Effective communication in English and Spanish, both verbally and in writing, required.
   » practice/involvement with and knowledge of Latino community required. 
   » Strong communications expertise and capability to establish positive working relationships with clients/families, health care providers, and other involved community members required.
   » capability to work independently and as a member of a team required. 
   » Must be able to maintain a flexible schedule – generally Monday through Friday office hours, but some evenings and weekends may be necessary. 
   » Must be willing and able to transport clients in personal vehicle while maintaining valid driver’s license, proof of automotive insurance, and maintain a good driving record.
   » Proficient working knowledge of email software, the Internet, databases, and common Windows-located programs, including Microsoft Office Word, Excel and PowerPoint required.  

Language expertise 

   » Effective communication in English and Spanish, both verbally and in writing, required.
   » capability to read, analyze, and interpret written materials, including reports, contracts, and proposals.  
   » capability to effectively present ideas and information in an organized and effective manner, including writing reports. 
   » capability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.  

Transportation 

   » Must have a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. 
   » Must be willing and able to safely transport clients in personal motor vehicle.
   » Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.

 Computer expertise 

   » Proficient working knowledge of email software, the Internet, databases, and common Windows-located programs, including Microsoft Office Word, Excel and PowerPoint required.  
   » Must be willing to attend training to advance technical expertise and maintain appropriate skill levels to execute job tasks.

WORKING CONDITIONS

Background check

   » Must pass a criminal background check, driving record check, and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.

Work Environment

   » Standard office environment with moderate noise level.
   » This position requires employee to drive between different sites by motor vehicle and visit clients in community locations as well as their homes.
