Organizational Unit Overview
ECU Physicians is the multispecialty group practice of the Brody School of Medicine. ECU Physicians has over 430 providers and sees over 700,000 patient visits per year. ECU Physicians is participating member in the Coastal Plains Network Accountable Care Organization and works closely with Vidant Health to improve the health of the citizens of Eastern North Carolina.
This complex organization faces increasing challenges to provide assigned nurses to serve as Health Coaches to assist patients with chronic disease management. The importance of chronic disease management is to not only improve the quality of our patient's lives but to also assist in meeting required quality measures.
ECU Physicians has obtained funding to hire 5 Nurse Specialist Health Coaches in 12 month time limited positions. The positions will be assigned to primary care clinics but will be centrally managed. The positions will be hired by ECU Physicians but will have joint reporting relationship with Director of Nursing for ECU Physicians and HealthAccess, Manager of Case Management. Vidant Medical Group through Health Access has had robust Health Coach Program and will assist in training and oversight through a management contract.
Job tasks
Provides individual and/or group health coaching located on protocols and best practices. Manages patient populations, including care transitions. Uses adult learning principles to facilitate behavior change and patient empowerment.
Collaborates with patient, caregivers, providers and other healthcare team members to develop a comprehensive plan of care. Utilizes motivational interviewing to support patient self-management expertise.
Supports medical home and care coordination principles; follows/completes assigned activities/protocols.
Communication and leadership abilities Clinical.
Participates in data collection/monitoring and formulation of process improvement plans. supervises assigned projects and/or programs. Serves as an expert clinical leader within ambulatory clinics by partnering with clinic Nurse Manager to review clinic dashboards and other quality measures.Demonstrates sound clinical judgment. Monitors patient's response to interventions and plans care appropriately.
Identifies educational needs of patient/family and provides instruction/referrals to meet self-management goals. Identifies priorities and organizes work accordingly. Follows evidence located practices and standards of care. Identifies educational needs of patient/family and provides instruction and community resources /referrals to meet patient healthcare goals. Utilizes motivational interviewing to support patient self-management expertise. Applies adult learning principles to facilitate behavior change and patient empowerment. Provides coaching and disease management services to identified populations. Demonstrates advanced nursing assessment and critical thinking. Functions as a change agent using effective communication expertise. Develops productive relationships with leaders, physicians, and clinical staff. Works collaboratively with other care team members to influence positive patient results. Supports team located care model.
Participates in data collection/monitoring and formulation of process improvement plans. Maintains Care Gap closure data indicating the number of care gaps closed so that downstream revenue can be tracked.
Maintains clear and effective continuing communication with providers. Methods of communication include documentation in the electronic health record, in basket messaging and oral communication to ensure continuity of care for patients.
Identifies priorities and organizes work accordingly. Completes chart documentation for patient visits within 24 hours of the visit. Acts as a resource for staff and providers in identification and use of Chronic Care Management Codes and HCC codes. Monitors the use HCC codes and updates patient charts at each visit.
Ensures proper billing codes are utilized for maximum reimbursement for Chronic Care Management.
CLINICAL SUPPORT POSITION – NOT SUBJECT TO STATE HUMAN RESOURCES ACT.
The ECU Brody School of Medicine is affiliated with Vidant Medical Center. Vidant Medical Center is the flagship hospital for Vidant Health and serves as the teaching hospital for the Brody School of Medicine at East Carolina University.
Minimum Education/practice
Graduation from state-accredited school of registered nursing and two years of nursing practice, with at least one year in the assigned specialty area. Must have a current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. All degrees must be received from appropriately accredited institutions.
License or Certification required by statute or regulation
Graduation from state-accredited school of registered nursing and two years or nursing practice, with at least one year in the assigned specialty area. Must have a current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. All degrees must be received from appropriately accredited institutions.
Preferred practice, expertise, Training/Education
Pediatric clinical practice.
License or Certification required by the Department
